- Utilizes the nursing process, for the performance of care, to assess, plan, intervene, and evaluate care to achieve prioritized patient outcomes within the scope of CNA.
- Contributes to, monitors and implements patient’s plan of care.
- Responds to patient needs in a timely fashion.
- Recognizes changes in patient's condition and notifies charge nurse in a timely manner.
- Serves as patient advocate.
- Participates as an effective member of the Care Team.
- Responds in a timely and effective manner to patient concerns and provides immediate interventions when possible.
- Provides written communication of concerns to necessary departments for appropriate follow up.
- Adapts to changes in workload and completes assignments while maintaining quality care.
- Initiates and performs duties which need to be completed although not directly assigned.
- Promotes teamwork through effective interpersonal relationships with all staff.
- Demonstrates initiative and flexibility in provision of care.
- Offers assistance to other team members as needed.
- Observes the patient at regular intervals, and documents on work sheet or verbally, information the Charge Nurse should know.
- Reports to the Charge Nurse any observations and/or symptoms indicative of patient’s changes and adverse reactions to treatments and/or response of complaints in a prompt manner.
- Reports care for assigned patients.
- Helps to set priorities when delivering nursing care.
- Maintains knowledge of location for needed equipment and supplies.
- Re-stocks supplies as needed.
- Reports equipment needing repairs.
- Cleans and stores equipment when no longer needed.
- Perform and maintain documentation.
- Documents accurate and ongoing assessments of patient status that reflect nursing interventions, patient responses, teaching and status of outcomes at discharge in care tracker.
- Completes shift end check prior to leaving duty.
- Communicate effectively.
- Follows approved shift report format when communicating patient information.
- Identifies and recognizes abnormal symptoms/changes in patient's condition, established priorities and takes appropriate action. Appropriately reports condition changes to Charge Nurse.
- Communicates and collaborates effectively with other health team members regarding patient condition, satisfaction needs and recommendations for meeting identified outcomes.
- Promotes and ensures patient and staff safety in all aspects of responsibilities.
- Utilizes proper body mechanics, transfer/lifting techniques to minimize injury to patient and self.
- Implements infectious disease controls as appropriate including isolation procedures, proper use of hand washing and gloves.
- Performs other duties as assigned.
