Overview
The Home Health Licensed Practical Nurse (LPN) provides services to clients in accordance with the State Nurse Practice Act (SNPA), agency policy, and accepted professional standards of practice. The Home Health Licensed Practical Nurse (LPN) provides nursing services in clients' homes in a manner consistent with Shepherd of the Valley's mission statement. This is a Hourly position with flexible hours.

Responsibilities:
Billing responsibilities:
- Release private pay/ VA and CSI claims from claims hold to formatted claims monthly.
- Release Medicare claims when all appropriate documentation is received
- Enters finalized cash receipts and updates accounts receivable ledger by customer for private pay.
- Obtains and mails invoice copies for customers.
- Answers phone inquiries on authorization requests and follows up.
- Mails correspondence to customers as necessary for collection and/or in order to update accounts.
Other responsibilities:
- Manage record requests and appeals
- Input employees and visit management in Sandata EVV
- Assist with nursing/HHA scheduling
- Medical record audits to ensure compliance with documentation
- Medicare pre-claim review management
- Participate in coverage of home health office when necessary
Intake Back-up Responsibilities:
- Communicate with referral sources to collect, and provide information regarding new and existing patients and enter information in the EMR
- Monitor and follow At Home with Shepherd patients in the hospital. Relay progress and updates to home health staff.
- Communicate with physician offices regarding patient care and relay this information to home health staff.
- Maintain and facilitate working relationships with hospitals, facilities, physician offices, and other referral sources.
- Answer requests for information about At Home with Shepherd from prospective customers and others
Home visits if needed:
- Will work with all age groups providing independent hands-on patient care in Trumbull County, Mahoning County, and Northern Columbiana County.
- Will provide wound care, manage complicated dressings, administer medications, and perform nursing procedures as required within the scope of practice.
- Provides independent hands-on patient care.
- Charts and documents all patient care according to state regulations
Requirements
- Must have a minimum of 1 year of experience in Home Health Care is required.
- Must be a Licensed Practical Nurse in the state of Ohio.
- Must have CPR certification.
- Must have a valid driver's license and be an insured driver with an acceptable driving record and reliable transportation.
