Program Summary
Project SHARE is an integral part of Shepherd’s House Ministries’ mission, serving as a vital entry point for relationships, services, and grace within Central Oregon’s unhoused community. The program is designed to build trust and collaborate effectively with local resources to meet the needs of vulnerable individuals. The Mobile Case Manager contributes directly to this mission by providing essential case management services, fostering trust, and supporting the holistic care of individuals experiencing homelessness.
Position Overview
The Mobile Case Manager plays a critical role in connecting individuals experiencing homelessness with essential resources, housing opportunities, and long-term stability solutions. This position conducts outreach to unsheltered individuals in the Bend area, builds relationships, and provides case management support to facilitate pathways toward permanent housing and self-sufficiency.
Key ResponsibilitiesOutreach & Engagement
1. Conduct outreach to houseless communities in Bend, fostering relationships and assessing individual needs.
2. Participate in regular outreach visits to camps and other locations to identify individuals in need of case management support.
3. Work alongside the Project SHARE and Shower Truck teams to deliver essential services such as hygiene supplies, food, clothing, and shower access.
4. Support volunteers and community partners in their efforts to assist unhoused individuals.
5. Develop personalized case plans focused on housing stability and other long-term goals.
6. Provide direct support to individuals and families by connecting them to housing programs, financial assistance, and other critical resources.
7. Advocate for clients with landlords, housing providers, and service organizations.
8. Provide education and guidance on navigating housing systems, including applications for affordable housing, vouchers, and emergency shelter placements.
9. Stay up-to-date on housing opportunities, community programs, and support services available in Central Oregon.
10. Collaborate with community partners, including local shelters, service providers, and government agencies, to maximize resource access.
11. Work closely with the Mobile Outreach Manager, Project SHARE Coordinator, Shower Truck Coordinator, and other team members to ensure a coordinated approach to service delivery.
12. Participate in team meetings and trainings to align on best practices and program goals.
13. Maintain accurate and timely documentation of client interactions, case plans, and outcomes.
14. Track data and metrics related to housing services and outreach efforts.
15. Provide regular reports to the Mobile Outreach Manager and other leadership as required.
Qualifications & Requirements
· Bachelor's degree in social work, human services, or a related field preferred, or equivalent experience in case management, outreach, or social services.
· Experience working with individuals experiencing homelessness, mental health challenges, or substance use disorders.
· Knowledge of housing systems, rental assistance programs, and community resources.
· Strong interpersonal and communication skills with the ability to engage and build trust with diverse populations.
· Ability to work independently and collaboratively within a team.
· Valid driver’s license and reliable transportation required for fieldwork.
· Proficiency in data entry and case management documentation.
· Ability to work flexible hours, including occasional evenings or weekends as needed.
Working Conditions
· Frequent travel between locations in Bend.
· Work environment includes outdoor outreach efforts in various weather conditions.
· Exposure to individuals in crisis, requiring strong emotional resilience and de-escalation skills.
Job Type: Full-time
Pay: $24.00 - $28.00 per hour
