Under the supervision of the Director of Low Barrier Service, the Redmond Center Case Manager (“Case Manager”) will engage relationally and collaborate with low-barrier shelter guests to create pathways to their health, recovery, permanent and/or supportive housing, and other means of positive life habilitation. The Case Manager will emphasize specifically on employment-related case management. Toward these ends, the Case Manager will learn participants’ stories and formulate specific plans for a healthier and more stable lifestyle. Additionally, the Case Manager Intern will participate in creating and maintaining a safe, supportive, and healing environment for participants to address physical, emotional, and mental needs.

Employment Status: Full-time, Salaried (exempt)

At the Shepherd’s House Redmond Center, we provide a safe and healing environment for those who need overnight shelter; meals and other basic support services; a nurturing and supportive community; and rehabilitative relationships and services. We seek to provide these benefits to protect the physical body and heal the spirit of trauma and indignity from longer-term afflictions. Our community-supported ministry seeks to engage communities, churches, organizations, and volunteers to join us in our mission within Redmond specifically, but also generally throughout Central Oregon. We seek to build alliances with such partners to serve the unsheltered and needy of Redmond and Central Oregon.

AREAS OF TRAINING AND RESPONSIBILITY

The Case Manager’s specific duties and responsibilities will be to:

  • Provide a welcoming, safe environment at the Redmond Center where guests are encouraged to access, and benefit from, day-use activities and navigation services.
  • Support and assist in creating and facilitating daily activities centered on experiential learning.
  • Engage with clients of the Redmond Center to learn their story and create an action plan specific to their needs.
  • Prepare and maintain client case files.
  • Record and report information including, but not limited to, Redmond Center occupancy, participation, connections, transitions, and exits. Specifically, be prepared to enter client point data into the HMIS system.
  • Guide and assist clients as they establish and navigate relationships with service providers and community partners. In support of this function, develop strong positive relationships in Redmond with public and private agencies, local businesses, civic organizations, potential employers and work sources, potential volunteer sources, and any other groups, associations, or individuals who would intersect with our guests and potentially be resources for them.
  • Guide and assist volunteers as they establish and navigate their relationships with the Redmond Center, our clients, and the Redmond Center team of staff and interns.
  • Collaborate with the Redmond Center team, service providers, and volunteers to identify and pursue new and creative ways to address chronic houselessness in our region. Specifically, establish and leverage relationships and sources of support within the Redmond community, to increase the availability of such pathways.
  • Engage with service providers to monitor progress, identify potential gaps in service and means to overcome apparent obstacles, collaborate on pathways to healing and habilitation, and coordinate regarding delivery of service and logistics to Redmond Center participants. Specifically, develop strong collaborative relationships with Redmond service providers.
  • Engage in neighbor relations to promote a positive standing of the Redmond Center.
  • Engage in other leadership responsibilities and duties as may be necessary.
  • Partake in providing safe, secure, and sanitary operation of the Redmond Center as a physical facility, through first contact interaction with appropriate facility service providers, as well as through guidance and/or supervision of appropriate SHM staff.
  • Fulfill any other duties and responsibilities as may be directed or assigned by the Director of Low Barrier Service, Director of Operations, or other applicable SHM leadership.

Job Type: Full-time

Pay: $54,080.00 - $58,240.00 per year
