Position: Hospitalist
Reports To: CMO
Schedule: Full time 80h/pp or Part Time Available
Position Location/Department: Sheridan Community Hospital
Job Summary: In collaboration with medical director and/or staff physician, provides care to hospitalized patients, focusing on improving their acute disease status and illness from the time of admission to discharge. Coordinates, reviews, and cares for swing bed hospital admissions through the rehabilitation process and facilitates the discharge plan. Serve as a leader, educator and researcher for patients, their families, and the community. Hospitalist service is a subspecialty of internal medicine or family, primary care medicine.
Essential Duties and Responsibilities:
- Provide diagnostic, preventative, and therapeutic health services to patients in the hospital.
- Evaluate and treat patients as the primary attending provider in consultation with primary care physician or collaborative physician.
- Review and coordinate the hospital's diagnostic, laboratory, therapeutic, and consultative resources for assigned patients.
- Follow through on treatments to ensure effectiveness and alter the course of treatment as indicated.
- Provide comprehensive medical care within the regulatory guidelines set by the State of Michigan and Critical Access Guidelines and Federal Regulations including prescribing and administering medical treatment and medications, performing diagnostic and therapeutic procedures; collaborate with physician in order to plan and coordinate patient care.
- Participate in the development, execution and annual review of the written policies/procedures governing the services of Hospitalist care; participate with his/her peers in a periodic review of the patients' health records.
- Effectively utilize consulting physicians and other diagnostic services.
- Perform such diagnostic procedures as appropriate per hospital policies at the bed side and procedures commensurate with experience and training.
- Interpret and integrate data to determine appropriate diagnostic and therapeutic procedures as needed.
- Perform complete history taking and physical exam and document in the chart per both the hospital medical record and CMS requirements. Collaborate with consulting and primary physician regarding management of patients' acute and long-term medical needs.
- Order appropriate laboratory and diagnostic procedures.
- Maintain a system for accurately recording and reporting patient conditions in the electronic medical record; follow the policies for timely completion of medical records, participate in annual review for quality and completeness.
- Collaborate with Care Management and recommend community resources to meet patient and family needs.
- Manage medical and surgical emergencies and perform the duties required of being on call after daytime hours.
- Attend required meetings; participate in committees as required.
- Participate in professional development activities and maintain professional affiliations.
- Maintain and ensure confidentiality of Protected Health Information (PHI).
- Perform other related work as required.
Education, Experience and Other Requirements:
- Must be certified as an Advanced Practice Provider (NP-C or DNP, PA-C) MD or DO in the State of Michigan and insurable for professional liability; demonstrate effective verbal and written communication skills, and ability to make quality independent decisions.
- Physical Demands: The job requires fifty percent (50%) of the time standing and/or walking, frequent lifting, pushing, and/or pulling loads up to 25 pounds.
- Accident/Health Hazards: The tasks performed on the job produce exposure to injuries (cuts, fractures, strains, burns, etc.) obtained in use of equipment, hazardous material or health hazards (based on contact with communicable diseases) that could result in permanent or long-term illness or injury.
