Responsibilities
Offers 3 days minimum per week, 8-10 hours a day to see patients. Adjustments of this schedule must be approved by Clinical Supervisor, Dr. Azhar, and Practice Manager
Obtains and maintains required malpractice insurance, state licenses and certifications
Provides mental health assessments to evaluate, diagnose and monitor patient symptoms and progress.
Demonstrates proficiency in conducting therapy sessions, developing treatment plans, and implementing evidence-based interventions
Provides exemplary psychotherapy services to individuals, couples, families and/or groups in person and via telehealth
Provides appropriate referrals to community resources as needed, including referrals to a higher level of care, including hospitalization
Conducts safety assessments and writes safety plans for at risk clients
Participates in weekly supervision with Clinical Supervisor – if required.
Reviews records provided by outside providers and updates electronic health records accordingly
Orders diagnostic tests as appropriate, reviews results, and adjusts treatment accordingly
Completes all necessary clinical documentation within 24 hours
Collaborates and consults with other clinicians, psychiatrists, care providers and families as appropriate
Must check work messages/work emails and reply at least one time a day to maintain effective treatment and supportive patient care
Attends scheduled staff meetings
Engages in ongoing professional development opportunities to develop skills and stay abreast of best practices in the field
Maintains up to date knowledge of DSM 5 and all relevant diagnostic information
Has familiarity with and comfort in treating mental health issues
Qualifications
Education Requirements: Masters or Doctoral degree in counseling, social work, psychology or related field from accredited institution
Licensure Requirements: Current licensure or eligibility for licensure in Illinois (LPC, MFT, MSW, LCSW, LCPC, LMFT, PsyD, etc.)
Professional Requirements: Malpractice Insurance $1,000,000/$3,000,000
Experience in private practice or related field, registered with CAQH, and paneled with insurance companies are considered assets.
