Responsibilities
- Provide patient care and assistance with daily living activities, including bathing, dressing, grooming, and meal preparation.
- Develop and implement individualized care plans in collaboration with healthcare professionals and family members.
- Assist clients with mobility and transportation needs, ensuring their safety at all times.
- Offer companionship and emotional support to clients, fostering a positive environment.
- Monitor and document changes in clients' health status or behavior, reporting any concerns to appropriate personnel.
- Provide specialized care for clients with memory care needs or those receiving hospice care.
- Support individuals with autism or other developmental disabilities through tailored activities and social engagement.
- Maintain a clean and safe living environment for clients.
Experience
- Previous experience in patient care, home care, or related fields is preferred but not required; training will be provided.
- Familiarity with care plans and the ability to follow detailed instructions is essential.
- Experience working with individuals requiring memory care or hospice support is a plus.
- Strong interpersonal skills and the ability to build rapport with clients and their families.
- A background in social work or nursing is beneficial but not mandatory.
- Cooking skills are advantageous for meal preparation tasks.
