We are seeking a dedicated Paraprofessional to join our team. The successful candidate will provide direct support services to individuals, ensuring their needs are met in a respectful and caring manner.
Responsibilities
Key responsibilities of the Paraprofessional include:
- Building trust and rapport with individuals, coworkers, and other stakeholders through effective communication.
- Identifying potential risks and behaviors that may lead to crises and implementing strategies to prevent or intervene.
- Assisting individuals in making choices and planning goals, while providing services to help them achieve those objectives.
- Providing support with daily living activities, including diet maintenance under the direction of a physician.
- Escorting clients to medical facilities, errands, shopping, and outings as specified in the person-centered plan.
- Assisting with communication, writing, typing, and researching information for clients.
- Closely monitoring clients' physical and emotional health, gathering information, and communicating observations to guide services.
- Following the written person-centered plan and carrying out duties assigned by the QP.
- Observing clients and environments, reporting unsafe conditions and behavior changes to the QP.
- Completing and maintaining records of required documentation, daily activities, and direct hours of service.
Requirements
To be successful in this role, you will need:
- A High School Diploma or equivalent.
- A valid Driver's License and proper Vehicle Insurance Coverage.
- MEDICATION ADMINISTRATION TRAINING: Successful completion of training.
- CPR certification.
- First Aid certification.
- HIPPA/OSHA training: Successful completion.
- Client's rights abuse and neglect training.
