- Interviews patients and family members to obtain information about home, social environment, family relationships, health history, patient's functioning and treatment goals.
- Explains patient's background, psychological factors, needs, and family dynamics to the team and participates in the planning of a comprehensive post-hospital care program for the patient.
- Collaborates with other health care professionals to develop, manage, and measure the effectiveness of the program.
- Assists patients and family with community integration and referrals to programs and services that will assist the patient in their home environment.
- Identifies eligibility and assists family in applying for entitlements. Investigates alternative sources of funding for uncovered services in the community.
- As clinically indicated, the DayRehab & OP Care Manager may provide psychotherapy services for patients to assist with the emotional reactions to their disability.
- Provide crisis intervention when necessary.
- Participates in professional organizations and activities; assists in the orientation of other health care professionals in understanding the services and purpose of social work
- Maintains patient documentation per organization requirements.
- Performs all other duties that may be assigned in the best interest of the Shirley Ryan AbilityLab.Reporting Relationships
- Reports directly to the Clinical ManagerKnowledge, Skills & Abilities Required
- Master's Degree in Social Work from an accredited university required.
- Current Illinois designation as a Licensed Clinical Social Worker (LCSW) required.
- Demonstrated understanding of the principles, practices, techniques and professional standards in the field of social work and psychotherapy.
- Knowledge of community resources, health care system and interdisciplinary functioning.
- Interpersonal communication, advocacy and analytic skills required to work with patients and families, and various medical professionals within and outside of the organization.Working Conditions
