• Interviews patients and family members to obtain information about home, social environment, family relationships, health history, patient's functioning and treatment goals.
• Explains patient's background, psychological factors, needs, and family dynamics to the team and participates in the planning of a comprehensive post-hospital care program for the patient.
• Collaborates with other health care professionals to develop, manage, and measure the effectiveness of the program.
• Assists patients and family with community integration and referrals to programs and services that will assist the patient in their home environment.
• Identifies eligibility and assists family in applying for entitlements. Investigates alternative sources of funding for uncovered services in the community.
• As clinically indicated, the DayRehab & OP Care Manager may provide psychotherapy services for patients to assist with the emotional reactions to their disability.
• Provide crisis intervention when necessary.
• Participates in professional organizations and activities; assists in the orientation of other health care professionals in understanding the services and purpose of social work
• Maintains patient documentation per organization requirements.
• Performs all other duties that may be assigned in the best interest of the Shirley Ryan AbilityLab.Reporting Relationships
• Reports directly to the Clinical ManagerKnowledge, Skills & Abilities Required
• Master's Degree in Social Work from an accredited university required.
• Current Illinois designation as a Licensed Clinical Social Worker (LCSW) required.
• Demonstrated understanding of the principles, practices, techniques and professional standards in the field of social work and psychotherapy.
• Knowledge of community resources, health care system and interdisciplinary functioning.
• Interpersonal communication, advocacy and analytic skills required to work with patients and families, and various medical professionals within and outside of the organization.Working Conditions
