Responsibilities
- Provide direct support to individuals with disabilities in home and community settings.
- Assist clients with activities of daily living (ADLs) to promote independence.
- Communicate effectively with clients and their families to ensure needs are met.
- Implement behavior management strategies as needed.
- Document care and progress accurately for reporting purposes.
- Drive clients to appointments and community activities as required.
- Collaborate with interdisciplinary teams to enhance service delivery.
Qualifications
- Strong communication skills for effective interaction with clients and team members
- Experience in home and community care, assisting with activities of daily living (ADLs)
- Ability to provide direct support and caregiving to individuals with disabilities
- Familiarity with behavioral health and behavior management techniques
- Willingness to learn and adapt, as no prior experience is needed
- LPN certification is a plus, but not required
