We are seeking a dedicated Resident Care Coordinator to join our team and help ensure the highest quality of care for all residents. In this essential role, you will be responsible for coordinating resident care, overseeing staffing, and collaborating with healthcare professionals to meet the evolving needs of our community.
Responsibilities:
- Supervise, hire, train, evaluate, and manage care staff, ensuring proper staffing and effective communication
- Develop and post staff schedules, and address any after-hours staffing concerns
- Coordinate staff orientations, in-service training, and performance evaluations.
- Maintain resident records, manage work orders for maintenance, and ensure compliance with OSHA and state regulations
- Lead care staff by modeling best practices in resident care, communication, and behavior management
- Oversee and monitor resident care, documenting changes in condition and collaborating with healthcare professionals, including physicians and nurses
- Assist in the management of medication systems, ensuring safe and effective medication dispensing
- Serve as a liaison between residents, families, and physicians, facilitating care coordination and family care conferences
- Ensure all care plans reflect accurate resident needs and provide ongoing updates as necessary
- Coordinate transportation for residents, and participate in management meetings to support the Executive Director
- Other duties as assigned
Qualifications:
- High school diploma or equivalent required
- CNA certification or prior experience in elderly care (residential or long-term care setting) preferred
- Proficiency in basic computer skills (Word, Excel, Outlook)
- Strong interpersonal, communication, and decision-making skills with the ability to build effective relationships with residents, families, and team members
- Ability to read, write, and speak English for clear communication with residents, families, visitors, and healthcare professionals
- Ability to maintain confidentiality and handle fluctuating community needs by re-prioritizing tasks as necessary
- Must meet all health-related state licensure requirements, including maintaining current CPR/First Aid and Food Handler certifications
- Completion of annual Continuing Education Credits
