What are the responsibilities and job description for the LVN Per Diem position at Shrine Home Health Care?

- Provide direct patient care, including administering medications, monitoring vital signs, and assisting with procedures
- Collaborate with physicians and other healthcare professionals to develop and implement patient care plans
- Document patient information, including medical history, symptoms, and treatment plans
- Educate patients and their families on healthcare topics, including disease prevention and management
- Assist with discharge planning and coordinate post-discharge care
- Perform sterile processing of medical equipment and supplies
- Conduct family planning counseling and provide reproductive health services
Experience:
- Must have a valid license as a Vocational Nurse
- Proficiency in medical documentation systems, such as eClinicalWorks
- Experience in sterile processing techniques
- Knowledge of physiology and medical terminology
- Familiarity with senior care and nursing home settings
