Job Overview
We have an opportunity for a part-time Social Worker to join our team in Salt Lake City. The Social Worker position requires understanding of common ethical and legal issues in social work practice in health care setting including: bio-psychosocial-spiritual assessment, use of the strengths perspective; client and family engagement in all aspects of social work intervention; case management/care management/care coordination/health-care navigation; discharge and transition planning; client concordance with and adherence to the plan of care; Advance Care Planning, Palliative Care, including pain and symptom management, Hospice and end of life care, identification of child abuse, trauma, neglect and exploitation; crisis intervention, facilitation of benefits and resource acquisition to assist client and families, including an understanding of related policies, eligibility requirements, and financial and legal issues, advocacy with other members of the interdisciplinary team and within the health care institution to promote clients and families decision making and quality of life, client, family, interdisciplinary, and community education; family systems issues, including the impact of health care concerns illness, and disease on family relationships; life cycles, and care-giving roles and support needs.
Schedule: Part-time FTE .5 (in-person)
Holidays: 9 observed holidays per year
Compensation range: The hired applicant's placement within the range is based on qualifications, professional experience and education.
Responsibilities
- Accountability for the achievement of optimal outcomes within th standards of practice.
- Ensures coordination of care with other Shriners Children's locations to best meet the patients needs as well as financial and clinical resources of SHC.
- Coordinates the plan of care to achieve desired outcomes and integrates the work of the health care team by coordinating resources and services necessary to accomplish agree upon goals.
- Advocates by supporting or recommending services for the protection of the patient's health, safety and rights.
- Collaborates by working with patients, families, caregivers, and the health care team locally and within the SHC system to jointly communicate.
- Demonstrates professionalism by emulating the standards of practice of care management guided by the Shriners Children's professional practice model FOCUSED.
- Arranges services among community agencies, physicians, patients, family, caregivers, and others involved in the plan of care.

Qualifications
Education
- Master's in Social Work, Required
Experience
- 3 years Social Work experience, Preferred
Certification
- Social Work license in state of practice, Required
- Case Management Certification (CMC, ACM or ANCC), Required
Additional knowledge and skills:
- Bilingual in English/Spanish preferred.
- Demonstrated competency in Microsoft Office Suite™ required.
