Shriners Children’s Chicago offers specialized care for a wide range of pediatric orthopedic, bone and joint conditions, including rare diseases and syndromes. Job Overview
Schedule: Full-Time Day Shift - 1.0 FTE
Shriners Children's Chicago has an opportunity for a Social Worker. In this role you will be responsible for providing assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care. Through this function, by joining the team, you'll have the opportunity to make an impact on our patient's experience. You'll help to provide continuity of care for positive outcomes in our family centered environment.
The pay range for this position is $69,617.60/year - $87,048.00/year. Compensation is determined based on years of experience and departmental equity.
Responsibilities
- Documents assessments, interventions, plans and evaluations in accordance with regulatory agency requirements and SHC policy.
- Coordinates the plan of care plan to achieve desired outcomes and integrates the work of the health care team by coordinating resources and services necessary to accomplish agreed-upon goals.
- Advocates by supporting or recommending services for the protection of the patient's health, safety and rights.
- Collaborates by working with patients, families, caregivers and the health care team locally and within the system to jointly communicate, problem solve, and share accountability for optimal outcomes.
- Arranges services among community agencies, physicians, patients, family, caregivers and others involved in the plan of care.
- Masters in Social Work
- Illinois Social Work license
- Case Management Certification (CMC, ACM or ANCC) required or the ability to attain certification within 12 months of hire or within 12 months of eligibility
- Three years of social work experience
- 5-7 years of social work experience
- Previous social worker experience in a pediatric setting
- Bilingual in English/Spanish
