A Patient Coordinator is responsible for the maintenance of ongoing communication with our current active patients and aides. In this important position, The Coordinator is responsible for management of current schedule for home care workers, while striving to ensure that patient/family are satisfied and receiving excellent customer service.

QUALIFICATIONS:
- Bilingual - English/Spanish
- Proficient in Microsoft Office Suite
- Working knowledge of HHA Exchange
- Highly organized, self-motivated, positive attitude, excellent customer service skills
- Ability to communicate effectively with employee, patients and their families
- Ability to define problems and tasks, collect data and establish facts, take action and resolve issues!
