JOB PURPOSE :
The Medical Respite Technician provides direct care, daily supervision, and facilitation of daily activities, for clients in the Medical Respite program at Siena Francis House. The Medical Respite Technician helps individuals reach their care plan goals, including overall improvement of health and wellness.
ESSENTIAL JOB DUTIES
- Intakes to include reviewing medical respite and shelter expectations,
- Provide direct supervision and support of clients in the medical respite program.
- Transport clients to and from appointments.
- Monitor the medical respite unit, to include ensuring a sanitary and clean environment.
- Complete accurate and timely documentation.
- Monitor client behaviors and elevate any changes in conditions.
- Works cooperatively / proactively with healthcare workers, case managers, clinicians, housing navigators, and shelter staff to help clients reach their care plan goals.
- Assist client with their discharge and transition plans.
- Completes all documentation and data entry as required, in a timely manner (Progress notes, Homeless Management Information System).
- Provides excellent internal and external customer service, behaving in a manner that cultivates collaboration, respect, and teamwork.
- Implements agency policy and procedures to effectively serve Medical Respite clients.
- Participate in applicable meetings, trainings, conferences, etc. as required both internally and externally.
- Communicate effectively in oral and written forms with clients, their families, and their significant others, coworkers, supervisors, other service agencies and the community remain culturally sensitive and respectful in all client and staff interactions.
- Adhere to all client confidentiality requirements & standards.
- Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards
- Perform other duties as assigned.
