About Us: Our mission is to promote individual dignity and support the health and well-being for at risk members of our community through compassionate services and resources. We do so through our; Supportive Housing Program, HIV/AIDS Support Services, Food Pantry, Street Health Medicine Collaborative, and our soon to launch short-term housing of Recuperative Care and Post-Hospitalization Housing.
Position: As a member of the Sierra HOPE team, the Housing Navigation, Coordinator, is responsible for implementing housing assistance and homeless prevention programs. The Housing Navigation, Coordinator, provides ongoing support to participants in housing programs, and is part of the Sierra HOPE Housing program team.
Reports to: Executive Director Job Duties
· Deliver high quality housing services that include local housing search, rental assistance, Fair Housing guidance and referrals, written client work plans, case follow-up and scheduling of additional Housing Resources and other services as needed.
· Conduct intake and initial assessment for applicants seeking housing assistance and enter into the Central Sierra Continuum of Care (CoC) Coordinated Entry System.
· Complete comprehensive assessment for applicants referred to or entering Sierra HOPE housing programs, complete all forms and collect documentation to verify eligibility and prioritize placement based on established criteria.
· Provide life skills education to small groups or individual clients using approved curriculum.
· Work with landlords and property managers to develop referral list, negotiate rents, and resolve problems.
· Participate in the CoC Coordinated Entry System committee meetings and conference calls.
· Maintain timely and accurate documentation in client files and in the Homeless Management Information System (HMIS).
· Prepare program performance reports as required by management or funding agencies.
· Attend regional association and community meetings and complete online or in person training events as assigned.
· Participate in annual Point-In-Time homeless count with CoC members and volunteers.
· Other duties as assigned.
Qualifications
· Degree in Social Work or related field, or equivalent work experience.
· Competence in crisis intervention and supportive counseling.
· Prefer two or more years of case management or housing navigation experience in a non-profit or other setting where the population served includes low-income families and individuals, people experiencing homelessness, and people with disabilities including physical, mental health or substance use disorders.
· Able to work well within, and meet the needs of, diverse populations and cultures.
· Possess good written and verbal communication skills, proficiency in computers and Microsoft Office programs including Word, Excel and email.
· Must be a team player, have good problem solving and decision-making skills.
· Meet all federal/state licensing requirements and/or program requirements necessary for this position, possess reliable transportation and a continuously valid California driver’s license, acceptable driving record, proof of insurance, and annual TB test.
Working Conditions
Work takes place in an office environment and involves local travel to meet with clients, inspect rental units, and conduct other business as needed. Requires occasional standing and computer work for extended periods of time, and some lifting required (20-40 pounds). This position is grant funded and conditional on Sierra HOPE receiving funding. This position may entail working after hours to attend community and/or business meetings or events.
Wages: $20 - $25 per hour. Compensation commensurate with experience.
Work Schedule: 20 - 30 hours per week. Position does have room for growth to full-time.
