Visalia Recovery Center is hiring a Case Manager. Our Case Managers play a vital role in providing needed resources to our patients, runs psychoeducational groups, and works collaboratively with the care team to provide high quality mental health treatment to our community. We are looking for an individual to join our team who thrives in a fast-paced environment, and who is seeking an opportunity to work with a diverse population of individuals who are experiencing moderate to severe mental illness.
Essential Functions:
- Provide high-quality mental health and SUD services (group treatment, case management, and consultation) to a caseload of 10-12 clients while also running 2-4 groups per week.
- Meet with patient, family members, and any other appropriate support persons to determine appropriate methods of meeting patient needs in community setting to meet mental health or academic goals.
- Assist patient in developing academic goals and plan that are supportive and mindful of mental health and SUD diagnosis to support patient in recovery plan.
- Assess each identified patient to evaluate individual needs for treatment.
- Review and evaluate client’s progress in relation to measurable goals developed through collaborate treatment plan between clinician and client.
- Identify speakers or groups within the community that will be appropriate collaborators, speakers, and/or linkages to patients in the program.
- Conduct individual, group and family collateral and case management sessions.
- Records interventions and interactions with patients, families and community agencies in progress notes in patients’ charts.
- Attend and participate in staff meetings and on-site staff training.
- Works alongside Discharge Planner and therapists to develop discharge plans for all assigned patients.
- Completes appropriate documentation in charts, treatment plans, clinical progress and utilization reviews.
- Assists with new admissions when required.
- Compliance with state and federal regulations, professional ethics codes, and insurance regulations.
- Maintain and submit all documentation and billing as it relates to client services.
- Provide the Clinical Director with timely reports on any unusual occurrences that might impact the overall program operation or the health and safety of clients or employees.
- Follow all reporting requirements for incidents of Child Abuse, Elder Abuse, Adult Abuse, Dependent Adult Abuse, and Tarasoff warnings per California reporting laws.
- Other duties as assigned.

Requirements
Qualifications: Certification as an RADT-I required.
Bachelor’s degree in in counseling, psychology, social work or related field preferred.
Experience: Must have a minimum of 2 years in the field of mental health/advocacy/intervention.
Skills: Documentation Skills, Intermediate computer skills in Microsoft Word, Excel, Outlook and PowerPoint. Ability to operate within Electronic Health Records (EHR) system. Analyzing Information, Decision Making, Research Skills, Verbal Communication, Written Communication, People Skills, Resolving Conflict, People Management, Integrity.
Certificates & Licenses: Valid Driver’s License, CPR/First AID
Other Requirements: Negative TB test results within past year, Medical Clearance to work within past year, Ability to pass pre-employment and random drug screenings
