PATIENT POPULATION :

The patient population served can be all patients, including geriatric, adult, adolescent, pediatric, and newborn. This also includes services which affect facility staff, physicians, visitors, vendors and the general public.

POSITION SUMMARY :

This may be a contracted service. The Occupational Therapist for DP / SNF reports to the Director of DP / SNF in collaboration with the Administrative Director of Imaging Services and PT for clinical issues. The Occupational Therapist is responsible for providing quality Occupational Therapy services to both Inpatients and Outpatients, according to standard Physical Therapy practice.

Must be able to work normal / scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call. Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours. As a designated disaster service worker you are required to assist in times of need pursuant to the California Emergency Services Act.

To perform this job successfully, experience in Healthcare Occupational Therapy is preferred.

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence if required.

Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, and percentages if required.

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

LICENSURE / CERTIFICATIONS :

To perform this job successfully, an individual must have and maintain current licensure as an Occupational Therapist in the state of California. Must have and maintain current BLS certification or obtain within orientation process, prior to hands-on patient care.

Responsibilities and Essential Functions :

  • Indicates Essential Function
  • Performs initial and ongoing evaluation of patients according to diagnosis, past history and physician referral. Is responsible for the assessment, treatment or care appropriate to the ages of the patients served.
  • Assesses patient's physical problems and needs for rehabilitation as determined by history and result of evaluative procedures. Draws conclusions regarding need for and results of Occupational Therapy procedures.
  • Provides and continuously modifies and adapts Physical Therapy programs and services according to patient's progress.
  • Responsible for maintaining accurate and timely patient care records to satisfy legal, hospital physician, and professional requirements, including weekly progress notes.
  • Responsible for assisting in the education and training of patients and their families to produce optimal patient response.
  • Demonstrates ability to provide comprehensive diagnostic screens / evaluation of loss of the ability for self-care, activities of daily living and / or ROM of patients referred to the department, and to determine the need for intervention and to develop individualized treatment plans.
  • Collaborates and integrates with other members of the patient's Interdisciplinary Team to carry out mutual patient care objectives as part of the Interdisciplinary Plan of Care.

8 Works closely with entire Physical Therapy staff to ensure all patients are receiving proper rehabilitation treatment.

9 Coordinates patient schedule with Physical Therapy and Restorative Nursing (if applicable).
