Shift : 6 : 45am - 7 : 15pm
Job Description : PATIENT POPULATION :
The patient population served in the DP / SNF is 21 years old and older. This also includes services which affect facility staff, physicians, visitors, vendors and the general public.
POSITION SUMMARY :
Under the direct supervision of a licensed nurse, the Certified Nursing Assistant (C.N.A.) performs individually assigned patient care activities based upon established patient care policies and procedures. Specific areas of responsibility include : monitoring of patient vital signs, providing patient hygiene and comfort measures, assisting with medical procedures, fulfilling nutritional needs, and maintenance of a record of the care provided as assigned.
Must be able to work normal / scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call. Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours. As a designated disaster service worker you are required to assist in times of need pursuant to the California Emergency Services Act.

LICENSURE / CERTIFICATIONS :
To perform this job successfully, an individual must have and maintain current licensure as a Certified Nursing Assistant (C.N.A.) in the state of California. Must have and maintain BLS certification or obtain within orientation process, prior to hands-on patient care.
Responsibilities and Essential Functions :

- Demonstrates support of the philosophy, policies and goals of Patient Care Services and the individual unit.
2 Documents care on the patient care record, accurately, consistently, and in a timely manner.
3 Prioritizes work activities to achieve optimal patient care.
a. Maintains quiet environment, e.g. provides warm blankets and monitors patients.
b. Preserves patient modesty and confidentiality in positioning, transporting and conversations.
c. Communicates with nurse pertinent patient information throughout shift vs. patient requests and documents on appropriate forms.
d. Interacts with patients to determine individual needs and coordinates with RN to accomplish.
a. Provides equipment and supplies for patient, room or procedure as directed (e.g. make beds, stocks supply carts, fresh water for patients, stocks soiled utility room).
b. Transports patient(s) to and from various hospital and / or nursing units with a pleasant attitude (e.g. Med / Surg to X-ray, ER to Med / Surg; discharging patients).
7 Interpersonal relationships demonstrate an understanding of personal responsibility to represent nursing and Sierra View Medical Center in a courteous, helpful and productive manner.
10 Adheres to hospital standards, policies and procedures relative to situation.
11 Available and functions as a resources to other caregivers.

12 Provides staffing support to the unit as requested by Director, i.e. scheduling flexibility, working extra shifts on short notice, low census days.
14 Responsible for general clean-up and tidiness of patient rooms, kitchen, refrigerator (check daily, clean weekly and prn), lounge, and countertops.
a. Equipment is properly prepared for Central Processing pick-up or reuse : bedpans, instruments, I.V. poles, IVAC pumps, VS monitors.
15 Compliance with all regulatory bodies : (DHS)
a. Documentation reflects compliance with established hospital policies and procedures
b. Demonstrates knowledge of role by adherence to NPSGs as it relates to area of practice.
