Job Description:
Clinical Case Managers at Sierra Vista Hospital play a critical role in coordinating and implementing case management strategies. As a Registered Nurse, you will work with all members of the healthcare team to ensure a collaborative approach is maintained in care and treatment of patients.
Responsibilities:
- Coordinate and implement case management strategies pursuant to the Case Management Plan
- Plan and coordinate care of the patient from pre-hospitalization through discharge
- Work with social workers, quality management coordinators, and risk managers to identify and address patient needs
- Assist with utilization activities and participate in performance improvement initiatives
Requirements:
- Current New Mexico licensure as a Registered Nurse or Licensed Social Worker
- Two or more years of hospital case management experience preferred
