About the Job
We are looking for an individual who is passionate about providing exceptional care and support to our residents. As a Social Services Coordinator, you will be responsible for assisting the Social Service Director/Consultant in planning, developing, and implementing social service programs that meet the needs of our residents.
- Assist in the development, administering, and coordinating of department policies and procedures.
- Review department policies and procedures, at least annually, and participate in making recommended changes.
- Assist in developing and implementing policies and procedures for identifying the medically related social and emotional needs of the resident.
Responsibilities
Your key responsibilities will include:
- Meeting with administration, medical and nursing staff, and other related departments in planning social services, as directed.
- Assisting in the development, administering, and coordinating of department policies and procedures.
- Reviewing department policies and procedures, at least annually, and participating in making recommended changes.
- Assisting in developing and implementing policies and procedures for identifying the medically related social and emotional needs of the resident.
- Participating in community planning related to the interests of the facility and the services and needs of the resident and family.
- Assisting residents in achieving the highest practicable level of self-care, independence, and well-being.
Requirements
To be successful in this role, you will need:
- A high school diploma or its equivalent; an Associate or Bachelor's Degree is preferred but not required.
- Prior health care experience is preferred.
- The ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
- The ability to write reports, business correspondence, and procedure manuals.
Work Environment
