Signal Health Group is looking for a LPN to join our home health team and provide care to our patients.
A Licensed Practical Nurse provides practical nursing skills within the scope of nursing practice standards in collaboration with RN case managers/supervisors, teaches and educates patients and their families according to the established professional plan of care.
Responsibilities
- Provide direct patient care in home settings
- Conduct thorough patient assessments and maintain accurate electronic health records (EHR).
- Administer and monitor medications.
- ensuring compliance with HIPAA regulations.
- Collaborate with healthcare teams to develop and implement individualized care plans.
- Educate patients and families on health management and support activities of daily living.
- Monitor patient progress and report any changes to the supervising nurse or physician.
