- Provide education and resources to patients and families regarding treatment options and post-discharge care.
- Monitor patient progress and outcomes, adjusting care plans as necessary to meet evolving needs.
- Advocate for patients by navigating healthcare systems and facilitating access to necessary services.
- Maintain accurate documentation of patient interactions and care plans in compliance with regulatory standards.
