Overview
As a Social Service Staff you will assist the Social Services Director / Coordinator in identifying and providing for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential during his / her stay at the facility and to assist in the planning for his / her discharge.
Responsibilities
- Assist residents and their families with social, emotional, and family issues.
- Assist in prividing timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.
- Participate in resident care planning and discharge planning.
- Serve as a general source of information for residents and their families
