Are you an experienced and compassionate Licensed Addiction Therapist with a passion for guiding individuals through their recovery journey?
Signet Health is pleased to be partnering with CHI Saint Joseph Health-Continuing Care Hospital in Lexington, KY. We are seeking a seasoned professional to develop a NEW substance use disorder program at our client’s (LTACH) Long Term Acute Care Hospital. In this role, you will play a pivotal part in supporting patients as they work towards lasting recovery and an improved quality of life. Responsibilities:
- Conduct diagnostic evaluations and assessments to determine patients’ mental health and substance use needs.
- Develop and implement effective personalized treatment plans tailored to each client's unique circumstances.
- Maintain accurate documentation of patient progress, treatment interventions, and outcomes.
- Facilitate group therapy sessions and educational workshops focused on substance abuse prevention and recovery strategies.
- Advocate for clients' needs within the healthcare system and connect them with additional resources as necessary.
- Excellent documentation and computer skills required.
- Regularly review and assess patient progress towards their treatment goals.
- Accurately and promptly enter all relevant treatment information into the electronic health record.
Requirements/Qualifications
- Master’s degree with current KY licensure as a fully licensed clinician (LCSW, LPCC) along with licensure in clinical alcohol and drug specialty required (LCDAC).
- Minimum of 2 years of clinical experience working with clients with substance use disorders (SUD) required.
- Excellent communication skills, both verbal and written, with the ability to build rapport with clients from diverse backgrounds.
