Silver Birch of Midtown is seeking an outstanding Resident Care Coordinator (RCC) to join our team!
What you’ll be doing
- First point of contact for families, visitors, physicians, and providers
- Maintains staff communication from shift-to-shift
- Manages medications, including cycle fill
- Completes monthly audits
- Manages new resident ALIS profiles
- Ensures incident reports are completed
- Ensures ADLs completed and documented
- Prepares resident service plans
- Manages staff schedule and covers open shifts, as needed
- Coordinates transportation for residents
- Trains new Med Techs

- Ability to actively cover on-call assignments
