The Pharmacist at Silver Hill Hospital will collaborate closely with healthcare teams to ensure safe and effective medication therapy management for hospitalized patients in a behavioral health setting. This role requires flexibility to adapt to varying workflows, handle complex medication regimens, and support the pharmacy department during periods of transition. Silver Hill Hospital Inpatient Pharmacy operates from 7:00 AM to 3:30 PM, Monday through Friday, excluding holidays. There is an immediate need for a part-time pharmacist, with future opportunities for growth.
Duties/Responsibilities:
Medication Management:
- Ensure prescribed medications are accurate and appropriate for patients' conditions.
- Collaborate with healthcare teams (doctors, nurses, dietitians) to review and adjust patient medications.
- Provide guidance on proper medication administration, including dosages, timing, and potential side effects.
Inventory Management:
- Maintain accurate, up-to-date inventory of pharmaceutical supplies and medications.
- Order medications and ensure proper storage and handling according to hospital policies and regulations.
Quality Assurance and Compliance:
- Ensure adherence to hospital policies, procedures, and regulations regarding pharmaceutical care.
- Participate in audits and inspections to ensure compliance with healthcare laws and standards.
Education and Training:
- Educate hospital staff on new medications, therapies, and emerging treatment protocols, as requested by the Director of Pharmacy.
Pharmacovigilance:
- Report adverse drug reactions or medication errors in compliance with hospital and regulatory guidelines.
- Work with regulatory agencies to report and monitor medication safety issues.
Collaborative Work:
- Communicate with physicians and healthcare providers to discuss treatment options, resolve medication-related issues, and contribute to patient care decisions.
- Collaborate with other departments, including nursing, to improve patient outcomes.
- Supervise pharmacy technician(s) as they perform their responsibilities.
Required Skills/Abilities:
- Must have the ability to function optimally in a stressful environment, and the ability to remain calm in emotionally charged situations.
- Knowledge of healthcare regulations, safety procedures, and compliance standards.
- Strong knowledge of pharmacology, drug interactions, and therapeutic principles.
Education and Experience:
- Doctor of Pharmacy (PharmD) degree from an accredited institution. Bachelor’s trained pharmacists with suitable experience will be considered.
- Licensed as a pharmacist in Connecticut.
- Behavioral health or health system experience preferred.
- Proficiency with Automated Medication Cabinets (AMCs) and Electronic Health Records (EHR).
Physical Requirements:
- Ability to move light objects weighing less than 20 pounds for short distances.
- Ability to sit at a desk and walk around for different periods of time.
