TITLE: Care Coordinator, LPN

JOB SUMMARY: The Care Coordinator is responsible for managing the daily routine delivery of resident care. An LPN care coordinator shall work under the supervision of the RN in the management and delivery of resident care; and work in collaboration with the RN to ensure resident safety and regulatory compliance. The Care Coordinator will ensure each resident receives the highest standard of quality care. Under the supervision of the RN, the Care Coordinator shall provide general observation and health supervision of the residents noting any changes in health and physical abilities, noting the need for medical attention or nursing services. He/she shall keep the family/sponsor, physician and the Administrator informed of changing conditions according to the wishes of the resident and family/sponsor.

QUALIFICATIONS: Must have genuine interest in Senior Adult care. Previous experience in an assisted living environment is highly desirable. Previous leadership experience is desirable. . Experience in working with persons with dementia is highly desirable. Enthusiastic, self-starter, and capable of using good judgment in making decisions. Must be able to communicate well with staff, residents, sponsors, and physicians.

EDUCATION: Completion of accredited nursing school with current and unencumbered licensure. Licensure as Assisted Living Administrator is highly desirable. Experience in administration and documentation of medications desired.

PHYSICAL DEMANDS: Standing, walking, bending, stooping, pushing and pulling. Must be able to assist residents with ambulation and transfers regardless of body weight without endangering the resident, self, or other associates. Specific vision abilities may include close vision; color vision and the ability to adjust focus.

RESPONSIBLE FOR: Managing the daily routine delivery of resident care.Initial assessment of new residents upon admission, when significant change in health status, or behavior occur and when monthly assessments identify problems in weight loss, falls, elopements, behavior symptoms or adverse reaction to drugs. Identification of resident care problems and collaboration with RN in formulation of interventions to address these successfully. Consulting with the Administrator on all issues of resident safety and health issues. Be available by phone to the Administrator for resident care concerns.

WORK PERFORMED:

  • Comply with ADPH Rules and Regulations for Assisted Living
  • Make daily assignments of resident care. Develop and maintain staff work schedule. Assist in the selection of new employees. Providing leadership for all staff assigned to the specialty care unit.
  • Perform on-call duties as assigned by Administrator and serve as Designee in the absence of the Administrator if requested.
  • Daily observation of residents noting changes in health and physical abilities that may require medical intervention. Keeping RN, Administrator, family and physician informed of changes in the residents condition.
  • Administer all medications on assigned shift and document in eMAR and record medication changes in each resident’s medical record and eMAR.
  • Assure resident's prescribed medications are available to the resident when they are needed.
  • Ordering and receiving medications according to established procedure through pharmacy supplier and company policy.
  • Check all batched medications and check all eMARs monthly.
  • Monitor resident’s monthly BP, pulse, weight and document.
  • Notify RN and Administrator of any changes in a resident's condition/behavior.
  • Complete an Incident Report per company policy.
  • Complete state reportable reports per company policy.
  • Work with Activities Coordinator in developing and conducting activities program that will be carried out daily.
  • Answer all call signals in a timely manner.
  • Screen prospective residents per ADPH guidelines for eligibility for admission into the specialty care assisted living.
  • Performs Initial assessments, PSMS, and Behavior Screens on residents per ADPH guidelines and requirements and communicate with attending physician, resident’s sponsor or responsible family member when a decline in health status or behavior occurs, or if the resident develops any of the following problems: Weight loss, Falls, Elopement, s/s of adverse drug reactions, over sedation or circumstances which contraindicated medications that have been prescribed, unmanageable, combative or potentially harmful behavior, any accident with injury.
  • Collaborate with community RN to develop a Plan of Care upon admission utilizing information provided by the sponsor, facility staff, physician's assessment and RN assessments.
  • Report any behavioral occurrences, which are un-manageable, including combativeness or suicidal behavior, to the Registered Nurse. Assist in arranging transfer for proper treatment.
  • Review all incident reports and collaborate with RN in the development of a Plan of Care to prevent future occurrence of incidents.
  • Conduct facility tours
  • Other duties as assigned.
