MDS Coordinator – Silver Memories Health Care (Versailles, IN)
Silver Memories Health Care is seeking a dedicated MDS Coordinator to join our skilled nursing facility in Versailles, IN. This position plays a vital role in ensuring accurate and timely resident assessments while maintaining compliance with state regulations and facility policies.
Position Details:
Schedule: Full-Time or Part-Time
- Full-Time: 24 hours per week as MDS Coordinator 16 hours per week as Floor Nurse.
- Part-Time: 24 hours per week, 8 hours per day, 3 days a week.
- Rate: Negotiable based on experience.
Position open to both Registered Nurses (RNs) and Licensed Practical Nurses (LPNs).
Key Responsibilities:
- Conduct and coordinate resident assessments according to required schedules or as needed for changes in condition, hospital returns, etc.
- Accurately observe, assess, and communicate condition changes to appropriate personnel.
- Ensure timely completion of assessments and Care Area Assessments (CAAs) in collaboration with Social Services, Activities, Dietary, and other departments.
- Complete MDS, quarterlies, CAAs, and care plans in the absence of other personnel.
- Develop and update individualized care plans based on resident needs.
- Prepare and distribute the MDS schedule to the interdisciplinary team.
- Transmit MDS data to the state per regulations and maintain accurate records.
Qualifications:
- Current RN or LPN license in the state of Indiana.
- Experience in long-term care as an MDS Assessment Coordinator.
