Responsibilities:
- Development and implementation of training content and continued education
- Following company policies and procedures to assist the client with their specific needs.
- Conduct thorough assessments for each individual client regarding the areas of the client’s needs, including health, psychological, social, and finances.
- Create comprehensive and individualized care plans while collaborating with clients, their families, and other involved parties
- Coordinate access to necessary services, resources, and support networks for clients.
- Documenting all contacts with clients using specific databases and verbiage specific to the program.
- Monitor and evaluate the effectiveness of the care plan, eliminating barriers, and adjusting as needed to ensure optimal client outcomes.
- Assisting clients in overcoming barriers using motivational interviewing techniques
- Maintain accurate and updated documentation of client interactions, progress, and service referrals.
- Collaborate with interdisciplinary teams as needed, such as healthcare professionals, social workers, and community organizations, to ensure proper care for clients.
- Participate in case conferences, meetings, and training sessions as needed or required.
- Provide guidance and support to incoming community health workers, assessing when to provide training refresher as needed.
- Staying current with CHW expectations and developing training as needed.
