Responsible for the delivery of safe, effective, and quality patient and family centered care in the assigned nursing unit. Roles during this phase of care focus on assessing the patient and developing a plan of care designed to meet the physical, psychological, educational and sociocultural needs of the patient and family. The incumbent must be flexible in regards to scheduled work hours and unit / work location as the schedule permits.
Essential Duties and Responsibilities :
Participates in plan of care that is individualized for the patient reflecting collaboration with other members of the healthcare teamPerforms appropriate treatments; within the scope of the LPN / LPN IV cert; as ordered by physician in an accurate and timely mannerReviews, documents and carries out all orders and patient / family education in a timely manner.Participates in admission, discharge and transfer process within the LPN / LPN IV scope of practiceDelivers care with appropriate age and cultural competence to specific patient populationsProvides individualized patient / family education customized to the patient and his / her familyTimely reporting and documentation of unexpected or adverse events.Initiates emergency resuscitative measures according to adult resuscitation and advanced life support protocolsMaintains confidentiality in matters related to patient, family and facility staffReports patient condition to appropriate personnel in a timely mannerMaintains current competency in nursing specialty by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societiesAdminister and document treatmentsDispense medications following policies and procedures and complete required documentation accurately and in a timely mannerMaintain complete and accurate patient recordsMaintain a safe and clean working environment in compliance with healthcare procedures and regulationsAdheres to infection-control protocolsAdheres to all hospital and departmental policies and proceduresMonitor and coordinate maintenance of equipment operation and inventoryCheck and maintain nursing supplies inventoryEducate and instruct patients and family members on discharge instructions and home care needsProvides mentoring, coaching and teaching to other staff as requiredAdapts and supports change in a positive and professional manner.
