What are the responsibilities and job description for the CASE MANAGER -MA Required Bilingual Preferred position at Sinai Chicago?
Conducts comprehensive case management activities designed to support and facilitate at risk pregnant clients access to health care and other appropriate services
- Monitor and document number of BBO women meeting adequacy of prenatal care as defined by Kessner Index.
- Conduct home visits according to contract performance standards
- Conduct appropriate Face to Face and other client contacts according to contract performance standards.
- Monitor and document number of BBO women who initiate prenatal Care within the first trimester.
WHAT YOU WILL NEED:
- Master’s Degree in Social Work Required or
- Master’s Degree in Counseling Psychology Required or
- Master’s Degree in Human Services and Counseling with specialization in counseling or Required
- Master’s Degree in Public Health (MPH) with specialization in public health, women’s health or reproductive health Required
*Minimum of 1 to 3 years experience in health, social or human service or community based setting.
*Strong organizational skills.
*Excellent oral and written communication skills required.
*Knowledge of basic computer operations required.
