What are the responsibilities and job description for the SR ABUSE CASE MANAGER position at Sinai Chicago?
Case Manager:
The case manager will be responsible for completing registration forms for SNAP customers.
- Responsible for creating a tracing form to identify registered customers and facilitate orientations.
- Facilitate client follow-up and assist customer in the resource center.
- Meet individually with the participant to analyze results, make referrals, and assist with career path selection.
- Responsible for accurae case notes.
- Track all supportive service activities.
- Facilitate workshops and participate in outreach activities.
- Monitor client to ensure that each client reach their 30, 60 and 90 day benchmarks.
- Submit documentation of clients' status to Program Manager on a timely basis.
- Participation is required in program training sessions
WHAT YOU WILL NEED:
- Minimum four (4) years of experience in the area of social services with a minimum two (2) years as a case manager. Minimum one (1) year working in training and employment setting.
- Bachelor's degree in Social Work, Sociology, Psychology or related field preferred.
- Excellent oral and written communication skills. Demonstrates ability to work independently.
- Basic computer skills required.
- Bilingual preferred
