GENERAL SUMMARY :
The Clinical Care Manager ensures each resident is provided and receives the necessary care and services to attain or maintain the highest practical physical, mental, and psychosocial well-being consistent with the resident’s comprehensive assessment and plan of care.
ESSENTIAL JOB DUTIES :
- Asses, evaluate, plan, implement resident care plans, and respond to resident needs.
- Ensure and oversee adequate staffing including meeting posting requirements.
- Ensure all residents are free from abuse, neglect, misappropriation of resident property, and exploitation.
- Participate in committees as assigned.
- Participate in the Infection Prevention and Control Program.
- Participate and contribute to the Quality Assessment and Assurance committee at least quarterly by identifying issues with respect to quality assessment and assurance activities, including performance improvement projects required under the QAPI program, as necessary.
- Active participation in CMS survey activity with adherence to regulations.
- Must be available to report to work in the event of an emergency.
- Participate in on-call rotation as scheduled by the Director of Nursing.
- Collaborate with the Director of Nursing on day-to-day clinical activities and strategies to assist with the nursing team's workflow and ensure residents' highest practical well-being.
- Participate in Community meetings including but not limited to Resident Care Plans, Medicare / Utilization Review, Weekly Risk Management.
- Participate in grievance investigations.
- Other job duties as assigned within scope of practice.

PHYSICAL REQUIREMENTS / WORKING CONDITIONS :
Frequent standing, stooping, bending, stretching, squatting; must be able to stand / walk for up to 70% of the work day; must be able to lift, transfer, turn, and position residents weighing up to 125 lbs. unassisted, or with the assistance of one for a resident weighing over 125 lbs. May be subjected to offensive odors and combative behavior. Must be able to lift and carry up to 20 lbs. frequently and 50 lbs. occasionally. May be exposed to communicable diseases including the HIV and HBV viruses. Must be able to work paying close attention to detail with frequent interruptions.
