Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. This role is essential in coordinating patient care, ensuring quality outcomes, and facilitating communication among healthcare providers. Duties
- Conduct comprehensive patient assessments to determine individual needs and develop tailored care plans.
- Coordinate care for patients across multiple settings, including medical-surgical units, ER, clinics, and Level II Trauma Centers.
- Monitor vital signs and evaluate patient progress to ensure adherence to treatment plans.
- Collaborate with interdisciplinary teams to optimize utilization management and enhance patient outcomes.
- Provide education and support to patients and their families regarding treatment options and care processes.
- Advocate for patients’ needs within the healthcare system while maintaining compliance with regulatory standards.
- Document all patient interactions and care plans accurately in electronic health records.
Experience
- A valid Registered Nurse (RN) license is required.
- Previous experience in geriatrics, acute care, or case management is preferred.
- Familiarity with vital signs monitoring, patient assessment techniques, and utilization management practices.
- Experience working in a Level II Trauma Center or ER setting is a plus.
- Strong communication skills and the ability to work collaboratively within a multidisciplinary team are essential.
- Proficiency in electronic health record systems will be beneficial.
