Job Summary
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. In this role, you will be responsible for coordinating patient care, ensuring effective discharge planning, and managing the overall health needs of patients. Responsibilities
- Conduct comprehensive assessments of patients’ medical histories and current health status.
- Develop and implement individualized care plans based on patient needs and preferences.
- Coordinate with healthcare providers to facilitate effective treatment and care transitions.
- Monitor vital signs and manage care for patients with infectious diseases or those requiring critical care.
- Oversee discharge planning processes to ensure smooth transitions from inpatient settings to home or other facilities.
- Maintain accurate medical records and documentation in compliance with regulatory standards.
- Utilize knowledge of physiology to educate patients and their families about health management strategies.
- Engage in utilization management activities to optimize resource use while maintaining high-quality patient care.
Experience
- A valid Registered Nurse (RN) license is required.
- Experience in medical-surgical nursing, critical care, or infectious disease care is preferred.
- Familiarity with discharge planning processes and utilization management practices is highly desirable.
- Strong communication skills and the ability to work effectively with patients, families, and healthcare teams are essential.
- Previous experience in a case management role is a plus but not mandatory.
