Position Overview
The Admission Coordinator is a critical part of the care team, which provides services to meet the social and/or emotional needs and activities that affect the patients’ ability to achieve their highest level of function. They participate in the development of patients’ comprehensive care plans and discharge planning. In addition, the Admission Coordinator develops policies and procedures to provide social services to patients in compliance with federal, state and local regulations ensuring compliance but focusing on requirements to meet patients’ needs. Responsible for performance improvement plans, tracking, and implementation. Performs operational tasks assigned by Nursing Leadership to support the Skilled Nursing Facility. Expectation is for all performed duties to be in accordance with Singing River Health System procedures and policies, accreditation organization, and governing guidance and publications for health care employees.

Education
Must meet the following minimum education requirements:
Must be currently licensed to practice in the State of Mississippi as a Licensed Practical Nurse (LPN); license must be free of any encumbrances.
Certification
Must complete and maintain BCLS certification upon hire and/or transfer.
Experience
One (1) or more years of social services experience preferred. Five (5) years’ experience in a long-term care / skilled nursing facility preferred.

Physical Demands
Work is moderately active: involves sitting with frequent requirements to move about the office, move about the facility, and to travel to another facility within the SRHS service area. Work involves exerting a negligible amount of force frequently to lift, carry, push, pull, or otherwise move objects, including the human body. Work involves using repetitive motions: substantial movements of the wrists, hands and/or fingers while operating standard office equipment such as computer keyboard copier and 10-key.
Work involves being able to perceive the nature of sound at normal speaking levels with or without correction; the ability to make fine discriminations in sound. Work requires close visual and acuity and the ability to adjust the eye to bring an object into sharp focus, i.e. shift gaze from viewing a computer monitor to forms/printed material that are closer to compare data at close vision.
Must be able to work for extended periods without experiencing undue fatigue. Must be able to work flexible hours.
Mental Demands

Special Demands
Must possess superior customer service skills and professional etiquette. Must possess proficient knowledge and ability to use a computer (must be keyboard proficient) and other office technology (i.e., telephone, fax, etc.). Must have working knowledge of MS Outlook, Word, and Excel.
Job may require traveling throughout the SRHS service area – with the employee providing his/her own transportation. Must have a valid driver license.
Work requires the ability to function independently, adapt to workload demands, set priorities, and understand and set goals. Must possess highly developed organizational, planning and management writing skills. Must understand the fundamentals of automated data processing, and be able to gain a detailed understanding of complex computerized and non-computerized information within allotted timeframe.
Must possess the skills to deal with families and their needs as they relate to the resident and the long-term care facility.
