What are the responsibilities and job description for the PATIENT CARE COORDINATOR position at Siouxland Community Health?
PATIENT CARE COORDINATOR
1 – Full-Time
Successful candidate must have solid experience & advanced knowledge of:
- Multi-Physician Family Practice experience preferred.
- Electronic Medical / Health Records (EMR) experience a definite plus.
- High School Diploma
- Current certifications preferred: LPN or MA (Applicants without certification, but have medical clinic experience will be considered).
- Current Mandatory Reporter certification.
- Solid computer skills.
Successful candidate must be able to perform primary functions of position:
- Work roster of patients recently seen in the emergency room or dismissed from a hospital admission.
- Ensure records regarding hospital visits are in medical record.
- Reach out to patients who have been discharged from the emergency department and hospital to determine how they are doing and if they need any assistance.
- Schedule appointments for ER/Hospital follow up when appropriate.
- Document all outreach efforts and patient interaction in Practice Management System.
- Communicate with the provider team any concerns or questions the patients may have.
- When appropriate, educate patients right place to receive care based on type of problem or condition they may be having.
- Ensure proper follow-up is completed, documented, and coded correctly for patients scheduled.
- Notify hospitals when records are received of non SCHC patients.
- Periodically check with hospitals to confirm that the SCHC primary care provider roster is up to date to ensure SCHC receives all documentation.
- Other duties and projects as assigned.
