Purpose
The Youth Case Manager provides oversight for an assigned group of residents. The person in this position develops nurturing, supportive relationships with residents. The Youth Case Manager will assist Program Manager with transitional living plans, coordination of services for youth and operations of assigned duties.
Position Requirements
- Bachelor's degree in social work from an appropriately accredited institution, OR Bachelor's degree in a human services field from an appropriately accredited institution and one year of directly related experience, OR Bachelor's degree from an appropriately accredited institution and two years directly related experience; or an equivalent combination of education and experience.
- Possess a valid North Carolina driver’s license.
- Dependable character and personal qualities as verified through interviews, reference checks, and criminal background checks.
- Requires moderate, physical activity at times.
- Maintain CPR/First Aid Certification
- Biennially complete and meet agency and regulatory rules for the following: criminal background check, responsible individual list, sex offender list, NC health and personnel registry, and physical.
- Requires 24 contact hours of training for staff development per calendar year.
Schedule weekdays and weekends.
Responsibilities (this list is not all inclusive)
- Treats residents with care and respect.
- Completes mandatory documentation within timelines established by applicable regulations.
- Empowers residents while demonstrating receptive attitudes and sensitivity to racial, cultural and ethnic heritage, sexual orientation, and gender identity.
- Develop relationships with residents to assist them with processing their issues and utilizing life events as opportunities for learning relevant life skills.
- Transportation of residents with appointments, meetings, etc.
- Become familiar with the history of each resident.
- Administer and review medication process. Review medication records and follow up with any issues.
- Represent Sipe’s at court hearings, team meetings, contact with family members, and youth social works in professional manner.
- Assist in the development and implementation of youth Transitional Living Plans.
- Makes contacts for youth regarding appointments, employment and post-discharge plans.
- Participate and represent Sipe’s with specific youth team meetings and referral sources.
- Maintain contact with youth counselors and therapists.
Job Type: Full-time
Pay: $16.00 - $17.00 per hour
