Job Summary :
The Resource Navigator is responsible for assisting clients in accessing and utilizing various community resources. This role involves assessing clients' needs, providing information on available services, and connecting clients with appropriate support agencies. The Resource Navigator will work closely with clients to ensure they receive the necessary guidance and support to improve their overall well-being in a five-county region.
Key Responsibilities :
- Client Assessment : Conduct thorough assessments of clients to understand their individual needs, challenges, and goals.
- Resource Identification : Identify and maintain up-to-date knowledge of local community resources, including disability support services, healthcare, housing, employment, food assistance, and other support services.
- Referral Services : Provide clients with information and referrals to appropriate services, agencies, and programs based on their specific needs.
- Service Coordination : Coordinate with external agencies and service providers to facilitate client access to resources and support.
- Client Advocacy : Advocate on behalf of clients to ensure they receive fair and adequate support from community resources.
- Follow-Up : Monitor and follow up with clients to ensure they have successfully accessed the resources and support services they were referred to.
- Documentation : Maintain accurate and confidential records of client interactions, referrals, and outcomes in compliance with organizational policies and privacy regulations.
- Collaboration : Collaborate with other team members, service providers, and community partners to enhance resource navigation services.
- Education : Provide clients with education and guidance on navigating complex systems such as healthcare, social services, and government programs.
- Outreach : Engage in community outreach activities to build partnerships and raise awareness of available services and resources (Transition Fairs, Choice meetings, Parent Nights)
Qualifications :
Strong interpersonal and communication skills.
Valid driver's license and reliable transportation for outreach activities.
