We are seeking an experienced Pharmacy Technician to join our Clinical/Medication Reconciliation team. In this dynamic role, you’ll work closely with a talented team of pharmacists, physicians, and nurses, contributing to the comprehensive care we provide to our patients. You’ll play an essential part in ensuring the safety and effectiveness of medication therapy by collecting detailed patient medical histories, compiling clinical data, and assisting in medication management. Your efforts will directly impact the well-being of patients, allowing them to receive the highest level of care and rehabilitation.

Key Responsibilities

• Assist pharmacists in medication therapy management by collecting patient medical histories upon admission.
• Compile and review clinical data, including lab results, for pharmacist evaluation.
• Support the Pharmacy team in providing exceptional care to patients, physicians, and nursing staff.
• Perform essential pharmacy technician duties, including technical support and inventory control.
• Contribute to medication profile management and assist in special projects as assigned.
  
  

What We’re Looking For

• Education: High School diploma or equivalent; Graduation from a CPhT program preferred.
• Experience: At least 4 years of pharmacy experience (inpatient preferred but outpatient considered). Proficiency in drug names and basic inventory control required. Basic computer skills needed.
• Certifications: Active PTCB certification and TN State Registration required.
• Skills: Strong communication skills, the ability to work well in a team, and an understanding of medication therapy management and customer satisfaction.
  
  

Physical/Mental Requirements

• Ability to move throughout the hospital environment, communicate effectively with team members, and manage manual tasks.
• Self-motivation, initiative, and attention to detail are essential in this role.
