Job Description : Backfill for CT Technologist position.
Job Requirements
Professional State License Following Applicable State Executive Orders and State Board Guidelines for Temporary Licensure, Emergency Waivers, and / or Licensure by Endorsement. Accepting of out-of-state licensure, when permitted by state executive orders.
ARRT Certification required.
Additional Details
Proof of vaccines or titers : MMR
Proof of vaccines or declination : Hepatitis B
Influenza Vaccine required between October 1 – March 31
One Step TB Skin Test / Tspot / TB QuantiFERON within past 12 months prior to start date
Driver's License / State ID required
Proof of vaccines or titers : Varicella
Annual TB Questionnaire within past 12 months
Physical / Health Statement within the past 12 months
12 panel (NO THC) drug screen in the past 12 months
7-year Criminal County Search, SSN Trace, National Criminal Database within 12 months
Catholic National Sex Offender Registry within 30 days prior to start
Catholic State Sex Offender Registry within 30 days prior to start
