Job Description for Sisters of St. Louis (SSL) Wellness Coordinator
Position Description
The part-time Wellness Coordinator ministers to 25 Sisters of St. Louis who reside in retirement centers in the greater Los Angeles /Claremont area by attending to their health care needs and supporting them in the transitions and changes that occur with aging. The coordinator collaborates with and reports to the Sisters of St. Louis Administrator and Leadership Team.
· To ensure living life in dignity and comport.
· To ensure activities and actions to prevent deterioration, maintain, and/or improve the status of abilities.
· To ensure end-of-life in comfort and peace.
Responsibilities
· Visit Sisters (on site) at least monthly.
· Assess functional abilities and document assessment data.
· Assess whether social, psychological, and safety needs are met.
· Drive and accompany Sisters to doctors’ appointments and advocate for them.
· Regularly meet with appropriate staff and management in the care facilities to exchange information and observations.
· Attend care-plan meetings as scheduled by the care facilities
· Facilitate smooth transitions to/implementation of new level of care as health condition or abilities change.
· Oversee the purchase of needed items for the Sisters (clothing, equipment, materials as needed for health care, comfort, safety, and sanitation).
· Communicate with the designated Health Care Agent (DPOA)/ next of kin.
· Maintain wellness records for each Sister.
· Meet monthly with the SSL Leadership Team to discuss condition changes of the individual Sisters and problems or challenges encountered in living places or circumstances.

Qualifications
Experience in nursing and/or Elder Care
Time Requirement: 24 hours a week
Hourly Rate: $30.00
Job Type: Part-time
Pay: $27.27 - $30.00 per hour
Expected hours: 20 – 24 per week
