What are we looking for?
The Licensed Practical Nurse Case Manager (LPN CM) will play a pivotal role in ensuring the health and well-being of the individuals we serve by conducting medication reconciliations, assessments, education and thorough documentation in compliance with state regulations. This role requires a detail-oriented, empathetic professional who can manage multiple responsibilities effectively.
Your overall essential responsibilities will be the following :
Key Responsibilities :
- Perform monthly medication reconciliations to ensure accuracy and safety of administration.
- Conduct comprehensive health needs assessments of clients regularly.
- Document client health records meticulously, adhering to state and agency standards.
- Maintenance of monthly medical notes, nursing assessments and other documentation related to medication use, general health status, and emergency procedures for assigned caseload.
- Provide education and support to staff, clients and families regarding health conditions and management.
- Participate in multidisciplinary team meetings to coordinate care, optimize client outcomes and share best practices.
- Collaborate with staff, healthcare providers, pharmacies, clients, families, and other stakeholders to ensure cohesive care.
- Maintain up-to-date knowledge of state Department of Disability and Aging (DDA) standards and ensure compliance.
- Monitor and evaluate the effectiveness of care plans.
- Ensure medical appointments are coordinated, referrals are addressed, and post-visit records are obtained.
- Assesses staff training needs regarding care delivery and medication administration protocols, and deliver real-time guidance to staff to minimize medication errors and gaps in care to improve quality of care and support delivered.

The hiring range for this position is $50,000-$60,000 annually.
