What are we looking for?
We are seeking a Service Coordinators for the Binghamton / Southern Tier Region to join our team. The Service Coordinator will provide participant-guided service coordination to individuals living in the community and enrolled in the Traumatic Brain Injury (TBI) Waiver and Nursing Home Transition and Diversion (NHTD) Waiver programs.
Essential responsibilities include the following :
- Establish and maintain regular contact, including face-to-face and in-home visits with program participants.
- Inform individuals and / or their legal guardians of their rights and responsibilities as a waiver participant, provide advocacy, and ensure that these rights are protected in service delivery.
- Complete assessments and facilitate the ongoing development of and updates to Individual Service Plans (ISPs).
- Facilitate regular Team Meetings, coordinate and monitor services among multiple providers.
- Respond to participant emergencies and address problems in service provision, support the participant in self-advocacy and problem solving.
- Provide referrals and linkages to resources, supports, and services.
- Maintain accurate and timely service records that meet regulatory requirements.
- Adherence to SJCS mission, core values, guiding principles, business standards and administrative practices.
The hiring range for this position ranges from $23 - $26 hourly and is dependent on location and experience.
