Southern Jersey Family Medical Centers is Southern New Jersey’s premier provider of primary and preventive care. Our interdisciplinary team consists of Psychiatric Nurse Practitioner, LCSW, LSW, and CMA. We provide a collaborative and comprehensive approach to assessment, treatment planning, service coordination, and advocacy for patients and their families.
Summary:
SJFMC is looking to hire Licensed Clinical Social Workers (LCSW) to join our collaborative team providing services to underserved communities in New Jersey. The LCSW will be provided an initial caseload of clients to be seen weekly for therapy sessions.
Position Responsibilities/Duties:
- Responsible for psychotherapy, supportive counseling and
therapeutic groups
- Facilitate psychoeducation and empowerment,
and the sharing of experience and knowledge
- Evaluate clients’
needs and develop personalized treatment
- Advocate on behalf of
the client and make referrals for appointments and assistance

Support each individual’s desire for independence and increased
self-confidence
- Document session content and generate notes
using EHR system
- Build and maintain positive relationships and
ongoing communication with the team, staff members, patients and
families
- Crisis intervention as applicable
- Provide
supervision as needed to LSWs
- Other duties as assigned to
support organizational and program goal
