Job Summary
Responsible for all Referral Center operations. Supervises Patient Registration and Reception. Triages and assigns referrals for timely and cost effective admissions. Demonstrates exceptional communication with, and education for, referral sources that result in an increase in admissions to HNW services. Utilizes knowledge of Hospice regulations and interpretations to assist referral sources in decision making. Creates and maintains systems that result in increased admissions and improvements to revenue.
Essential Functions
Collaborative communication with the Clinical Manager, MSW/Chaplain Supervisor, Clinical Liaison and Community Liaison. Demonstrates agreement with Liaison personnel that the Referral Center must meet the needs as discussed with the Liaison personnel. Consults with Medical Director regarding eligibility questions, atypical interventions, additional data to support terminal diagnosis, etc. Communicates efficiently with referral sources to verify eligibility, educate regarding appropriateness, coordinate initiation of care, request additional supporting data and/or suggest alternatives when necessary. Responsible to assure appropriate and timely admissions. Confirms payer coverage is verified and documented prior to scheduling an admission visit. Responsible to meet all requirements per the particular payer that guarantees maximum payment to agency for hospice care. Demonstrates knowledge of chronic disease management. Demonstrates knowledge of hospice and palliative care. Demonstrates ability to identify need for improved systems. Plans and implements improvements. Revises and maintains new systems. Considers effects on referral sources and agency personnel/systems prior to implementation. Meets or exceeds all expectations for the referral and admission process. Expected outcome is increased efficiencies and increased admissions with decreased losses from lack of viable payer, unnecessary assessments, etc. Utilizes the electronic medical record for all referral communication. Applies knowledge of hospice philosophy and care in decision making as it pertains to referrals and admissions in hospice care. Demonstrates assertive communication to assign admissions and informational visits to clinical staff, in conjunction with Patient Registration. Supervises Patient Registration and Reception. Actively mentors to assure exceptional performance and success. Develops and maintains systems for Recalls, Insurance Verification, Maximum Utilization of Reception, Data Analysis and others as identified by manager or designee. Shares in Administrator on Call rotation (following first six months in position).Attends committee meetings and other forums as appropriate and approved for communication, planning and organization. Assists in development and revision of appropriate Protocols, Policy and Procedure. Adheres to all SRH and HNW policies. All other duties as assigned.
Education
RN from an accredited school of nursing, BSN preferred.
Experience/Training
Two years medical/surgical experience. Hospice and supervisory experience preferred.
License/Certifications
Current Washington State RN License required.
Other Skills
Knowledge and understanding of medical terminology, hospice eligibility and health insurance requirements. Strong verbal, written and interpersonal communication skills required. Proficient computer and office equipment skills. Ability to maintain positive demeanor and exercise good judgment in stressful and emotional situations. Excellent customer service skills required.
Physical Demands and Work Environment

While performing the duties of this job, the employee is regularly required to sit for long periods of time; when working in office. Repetitive tasks such as typing, sitting, answering phones, and interacting with computers and computer systems most of the day is a function of the position. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to focus. This position requires working in an indoor, environmentally controlled environment when in the office.
