What are the responsibilities and job description for the Hospital Liaison / Marketer position at Skilled Nursing Care?
The Hospital Liaison/Marketer serves as the primary point of contact between our nursing homes and families, hospitals, referral sources, and the community. This role is responsible for facilitating and coordinating patient referrals and admissions across our nursing homes through strong relationship-building with healthcare professionals—including Social Workers, Case Managers, Discharge Planners, Physicians—as well as community organizations and partners.
Working closely with internal Admissions Teams, the Liaison/Marketer ensures all clinical and financial information is accurately communicated and assessed. This position plays a key role in identifying opportunities to grow referral volume, enhance collaboration with hospital partners and community networks, and support overall census development.
Key Responsibilities:
Conduct daily hospital visits to maintain visibility and engagement with healthcare teams.
Build and maintain strong relationships with Social Workers, Case Managers, Discharge Planners, Physicians, and key community contacts.
Serve as the main liaison between hospital staff, community partners, and our nursing homes, facilitating clear and timely communication.
Participate in daily rounds as appropriate; review patient medical charts to assess discharge plans, clinical needs, and financial coverage.
Represent our nursing homes in a value-added capacity by attending patient rounds, providing in-service presentations, hosting CEU events, and offering outcome-based clinical feedback.
Follow up on all acute readmissions and provide status updates to internal Admissions Teams.
Generate new business and referral sources by nurturing current relationships and identifying new opportunities with healthcare professionals, community organizations, and facilities.
Coordinate and execute in-service and promotional events at hospitals, within the community, and at our nursing homes, in collaboration with Admissions Teams and nursing home Administrators.
Promote nursing home capabilities, clinical outcomes, and programs to hospital leaders, physicians, and community stakeholders.
Identify areas of growth and expansion within the market, including community-based outreach.
Serve as a key resource for both internal and external teams to support the referral and admission process efficiently.
Collaborate with the Corporate Business Development team to set metrics, goals, and quarterly marketing objectives.
Assist in developing annual marketing strategies and team objectives.
Qualifications:
Education & Experience:
Minimum of 2 years of experience in healthcare marketing or liaison roles.
Long-term care experience is required.
Strong knowledge of Medicare, Medicaid, and third-party billing systems.
Familiarity with PDPM, ACO models, and various payer systems (HMO, commercial insurance, workers' compensation).
Ability to interpret medical terminology and discuss clinical cases with physicians and healthcare professionals.
Work Environment:
Operates in a healthcare setting with frequent walking between hospital departments and patient areas.
May be exposed to infectious diseases, traumatic medical scenarios, or deceased patients.
Must be available to work beyond regular hours, including weekends and holidays as needed.
