What are the responsibilities and job description for the MDS Coordinator position at Skilled Nursing Care?
Our Skilled Nursing Facility is looking for a Five Star MDS Coordinator!
A primary focus of our MDS Coordinator is the resident assessment process, which includes individual assessment, MDS assessment, and resident plan of care. The MDS Coordinator manages the resources provided to residents and coordinates the entire continuum of care.
MDS Coordinator's responsibilities include:
- Provide accurate documentation of all services to assure accurate reimbursement from all payor sources.
- Ensure that needed resources are available and that quality care is delivered to all residents.
- Coordinate the development of a written care plan and assessment for each resident and review and revise as appropriate.
MDS Coordinator's Requirements:
- Must have a current nursing license.
- Must have Long Term Care experience.
- Must be knowledgeable of long-term care nursing and medical practices and procedures.
- 1 - 2 years of MDS experience
Skill Required:
Communication: Must be able to effectively give and gather information from medical staff about patients and communicate with family members about patient care.
Management: The MDS coordinator manages the nursing staff by monitoring patient care and performing management tasks such as monitoring patient records.
Organization: Strong organizational skills are required to track patient records and evaluate the care they receive along with managing information for medical insurance and other financial information.
Leadership: Leadership skills are necessary to lead meetings and ensure the nursing staff meets patients' needs while receiving support for their work.
